Behind the scenes at the UK's most remote hospital, the Gilbert Bain in Shetland.

In this episode, paramedics and the coastguard rescue team help a six foot nine inch Norwegian nicknamed Tiny off his tall ship. The sailor was on his way to the Shetland Folk Festival, but having a bad ankle fracture means he’ll spend the night in hospital instead.

Nurse Amanda treats an old school friend, fisherman Neil, who was also hoping to make it to the Folk Festival. He arrives in A&E with excruciating backpain.

Specialist paramedic Emma Stevenson takes time off to play her fiddle at one of the festival’s more unusual venues.
